Grace period Tier 5

Hi,
I read on the GOV.UK web site that there is a grace period of 28days for a Tier 5 visa holders. But then read on different websites that it was lifted as a possibility.
What is the valid information concerning this issue?

Also is it possible to apply for a new Tier5 before the expiration date of the current one, due to the circumstance that I’m changing my employer.

What category of tier 5 do you hold?

What 28 grace period exactly are you referring to?
