Parents visit expire feb 2021 cannot go back due to medical reason

Hi.
This is question for my friend.

His parents old age 80 plus.
No family ties back home .
Both son and daughter in UK settled.

I want to apply parents dependent visa.
Currently they are on visit visa expires Feb 2021

Can I apply any further leave to remain

This will be Adult Dependent Relative visa which from people's experiences appears to be the hardest UK visa to succeed in application.
How many months have they spent in UK already?

Was the visa granted without ties back home?

The Adult Dependent Relative visa is really hard and you can end up in a difficult situation with your parents at your charge but technically without status in the UK (this means no NHS or other help). So please tread carefully here.

If the application fails, they will never get a visitor visa again.
