The Appendix 2 is already incorporated into the online application and hence no need to fill it.

You can go with the scanned documents on USB to the appointment so that should there be any missing or illegible document found you got a last chance to upload it.

on appointment day in Pakistan do need to take all the documents? You can self upload your supporting documents which is much better because there's no limit with regards documents you can submit.
My opinions should not be constituted as an immigration or legal advice.

Don't take anything on USB stick, they will not accept that. My advice is to print all your docs and they can scan it in case something has been lost or poor quality.
