EUTR1A application form

HI All,
Hope to find some advice on the matter below.
My partner and I are filling out an EUTR1A form and in section 7 there is a confusion.
I am a self employed (contractor) and have filled out the "EAA national self-employed" part.
What I don't understand is do I have to fill out Appendix A - EAA national's declaration of employment part as well.
As that would mean I will be filling out the employee part and employer part as I am self employed and by my understanding work for myself.

Could someone clarify that please.
