URGENT info required on T1G Extn in country application

Hi - I came to UK under workpermit and shifted to HSMP. Currently I am in UK on HSMP VISA (as per post 5 Dec 2007 guidance). I am planning to submit application for my extension of stay. I have few questions and appreciate your help

Main Applicant - Application
1. D4: Please enter the reference number on the entry clearance or visa which can be found in the applicant’s passport or travel document.

I have both EC and residence permit in my passport. Which number should I mention.

Dependent Application
1. D5: Reference number on the entry clearance or Visa: As I mentioned earlier, in my wife's passport, both EC and Residence permit are available. Which number should I mention here.

2. My daughter is born here and I am not sure what should I answer for question D1: Did dependent obtain EC or Visa before entering the UK?

3. J2: Has the main applicant received notification that their application for leave to remain under the Points Based System has been successful?

- I think the answer is NO please clarify

4. For dependents do I need to submit the following as a proof

a. Marriage Certificate
b. Birth certificate (child)

1. main applicant: entry clearance, the red number
1. dependent: entry clearance, the red number
2 duaghter : answer: no
3. sorry, i dont know
4. yes, u have to submit relation certificates...

Regarding the question 3: J2: Can some one please clarify?

I have one more question

Do I need to need to submit multiple checks (i.e., separate cheques for main applicant and dependents ??

2. B13: Points Based System migrant reference number - Can I leave this blank as I was not under Tier 1 (G)

3. Under Section 3C - T9 - Do I need to mention bank contact details as well along with my employer contact details?

Not necessary, but for peace of mind if mentioned.
