Quuery for filling VAF9 form

Hi All,

Please help me on the below queries.

1. The given name in my passport is "Anish Bushan" and Surname in passport is Blank. So should I fill in Part 2 -> 2.1 in VAF9 Given name (as shown in passport) as "Anish Bushan" ? and leave Part 2 -> 2.2 in VAF9 Family name (as shown in your passport) BLANK? Please confirm is this correct.

2. My name in the degree certificate is "Anish B". I am getting an affidavit stating that both the names "Anish Bushan" and "Anish B" belong to me. So should I have to fill in "Anish B" under Part 2 -> 2.3 in VAF9 Other names ? Please confirm.

Logically speaking your surname/Family name is "Bushan" and your given name is "Anish" which is your first name as well. I wonder how the hell your passport doesn't show your surname as "Bushan". Are they That DUM that they cant even separate the 2 them selves... Daaamm.. Thats DUMM !!

But anyways... Plz download the "PBS guidance notes" specifically for VAF9 form from this link

And read the point 2.1 and 2.2 according to which your name must be same as mentioned on your passport. So if your surname is not mentioned on your passport then leave it empty on the form.

Moreover fill your other name "Anish B" in form where it is mentioned "Other Names".
