One of the parents should sign the form (as normal).

To further clarify - the form is for PBS dependants. In addition, for the T-2 & T-4 the form is also an application for Biometric Immigration Document. Also note that they have revised the fees and the forms too have been amended accordingly.For dependants of Tier 2 and Tier 4 Migrants, this is also an application for a Biometric Immigration Document.
