Name change

Dear friends,

I am applying for Tier 1 extension. Need help regarding Question-B5 of dependent form i.e. name change

My wife surname has already changed in passport 3 years ago. at the time of initial application we left name change Question blank.

Now for extension, do I need to complete this Question and provide any evidence?
