I would like to apply for the PSW in person at a PEO, and am trying to book an appointment online.

On the first page, I am required to select the type of application from a list below:

EEA1 (FMOP)
FLR (BID)
FLR (IED)
FLR (M)
FLR (O)
NTL
PBS T2
PBS T4
PEO Tier 1/5 Biometric Desk - Online
SET(F) under 18
SET(O) excluding long residency
SET(O) on long residency
TOC

Which is the correct application type?
