PEO appointment booking form

Hi

I have booked an appointment at PEO.
After booking completed, I saw a page saying, ,print the official form and sign ,

Does it mean the application form SET(O)?

or is there anyform we get after normal appointment booking which can be signed to submit at the PEO?

how did u book it? By phone or Internet ?

i have been trying since last week and everyday it says there are no slots till Mid Feb.
