Two Evidence Req. to be produced for Self.Empd , HSMP to T-1

I m going to apply for my HSMP extension into T-1, after couple of months. Registered as Self Employed Sole Trader. I need to know what documents to be produced as two evidences of previous earnings as self employed. Where i m providing services to the people and charge money for that which is usually paid by cheques and i deposit in my business a/c. Tax Return for that period is due to but not has been submitted.
I'll be very thankful if someone can guide me.

List of documents I have provided:

1. Certificate of earning (issued by my accountant on his letterhead confirming my gross and net earnings for the required period)

2. Balance sheet and Profit and Loss (again on accountant letterhead though not required but I asked him to provide these)

3. Invoices printed on plain paper (I trade on ebay and provided all sales records)

4. Bank Statement showing deposits from paypal

5. Paypal statement confirming withdrawals


I would suggest to submitt atleast 1 - 4

thanks Ali......info u provided is really helpful to me......
wud it b fine if i provide only 2 evidences (1)Bank Statement (2)Certificate of earnings from accountant showing gross and net earnings.

You need to show Bank Statement as source one

in terms of certificate of earning that is source two but there could be questions as Customer information or invoices
Also provide them with Balance Sheet as it has proven in the past to be vital.

better be prepared with extra supporting docs rather than just meeting the 2 required...add at least another one...If i were you, follow the suggestion provided earlier by other who commented. good luck.
