Tier 1 Entrepreneur - Dependent Visa Application

Hi Admins,

Please I need your help. I am on Tier-1 Entrepreneur Visa 50K and have my limited company. My visa is valid until September 2019 when I will apply for my ILR.
I want to apply for my wife dependent visa since she is outside UK. I have the below Questions:

1- Is this the link to apply for Tier-1 Entrepreneur Dependent visa?

If yes then what options do I need to visit when it comes to ''Visa Type''. There are 3 drop downs so which one should I select?

2- My T-1 ENT visa is until Sept 2019 so my wife will get her dependent visa until that my visa expiry. So do I still need to Pay 3 years NHS?

3- Do you know the Fee for the Tier-1 ENT Dependent Visa?

4- Do I need to fill this form as well along with online Tier-1 ENT dependent visa form?

You have to apply through visa4uk site from here
From dropdown select work and then t1 indpnd
U have to pay minimim ihs 2 yrs
Fee is £1389
Other than that you dont need to fill anything
Once u submitted everyhing here select appinmt date and ask ur wife to take all documents, they will keep passport original and copy everything else..
Should take 10 days to get visa iA...

I am applying for my wife (T1 Ent Dependent Visa)
I am confused about maintenance fund, how much do I have to show :
Just £630 for dependent?
or
I have to show £945 for myself too

Means total: 630+945

Apparently its 630 for dependent only
But suggested good if u jeep both

I have checked with a solicitor, They have told me it is just £630

Can you please write down a list of supporting documents related to sponsor income, should i have to submit CAR, VAT receipts, P60 Payslips? Please update
