Tier1 Entrepreneur PBS Dependent Extension consent form query

Hi All,

I hope you're well and staying safe.

I'm applying for my wife visa extension (PBS Dependent) and i don't see an option to download and sign consent form. Is that normal or am i missing something? Nothing about consent form in the document check-list or when completing online application apart from Declaration as applicant.

I had received my ILR via Tier 1 Entrepreneur 5-years route.

Just for confirmation, my wife will sign part 1, I will sign part 2 as utility bill on both of our name. Do I need to sign part 3 as I have maintenance funds in my personal account? Will I be 3rd party sponsor for my wife?

Pls also will latest joint utility bill, wife’s personal statement, Nhs registration, NI letter & sons date of birth certificate enough for living together proof?
