Tier 1 PBS dependent ILR application cohabitation req

Hi,

My wife came here as PBS dependent on tier 1 in June 2013. Now we are about to apply for her ILR using 5-year route. she is not working
I am now a British passport holder.

I have a few questions and few things to confirm. Any help would be appreciated.

I am submitting an application with the following documents.

From my wife (applicant):

Set O form as PBS dependent.
Life in the UK test
English language test
2 photographs
BRP
Passport


From Me:
3 months bank statement
3 pay slips.
My British passport



I have the following questions.

1) Do I need to attach my cohabitation proof for last 2 years (or any period). ? from form it looks like it is not needed. But not sure.
* How many different documents do I need? For me and my wife.
2) We have selected the same day service. Do I need to go her with for that as her sponsor ? someone told me it is needed but not sure.
