I want to apply Spouse visa for my wife. Does she need to appear for IELTS even if she has done O'levels with Grade B (English) & Grade D (English) in A'levels from Pakistan?

What 'spouse visa' exactly are you applying for??? There is more than one category!!!

You appear to be jumping across different immigration categories so you need to be more clear in posting please and post in the correct sub forum, this one where you posted is for Tier 1 General Dependent visas!!!
