PSW DEPENDENT HELP??

Hi,
I am on psw and my wife is dependent on me. we have a 4 month old baby born in uk and now we need to visit our home country so we need dependent visa for our baby. I am confused about which form i need to fill. I though PBS dependent form is the right one. And i need to know how much is the fee for premium one day service??

I have question about psw depedent visa . actually mid of December my wife apply for dependent visa and ukba refused it and when she apply that I got 7 months visa and now I want to apply again so I am just confused my visa left only 5 months so I wanted to know that can my wife apply for dependent visa .please help me I don't have enough time coz my visa goin expire in June 2014 . please help me as soon as possible.thanks

any one know that is there any time period for psw dependet visa . I have only 5 months visa left so can we apply dependent visa from Pakistan .
please help me please .
