Uplift ratio

Hi guys,
I am facing a confusion. Do we mention the salary with our without the uplift ratio in form? Search shows either Yes or not sure. Can any one please confirm..

Hi

i actually did both by squeezing in xxx * 5,3 = xxx in the tiny box
i also showed the VFS councellor and they said it is fine.
