Urgent help with Naturalisation application

Hi all,
Hope you all are safe. This is my first post and if any one surely can answer these questions, it would be very helpful for me.

Under immigration history details
'Name in which you made the application.'
** My question is, Shall I write my previous visa type? For example, Tier-4 General student Visa! Or do I need to write my Full Name!

Under English language
Do you have a previous visa, entry clearance or leave to remain where you had to prove your knowledge of English?

**I have done my IELTS in UK in 2012 before my university admission. After my university, I never had to do any English test anymore. So my understanding is, this question answer should be YES.

** When I say YES then they ask Tell us the previous visa, entry clearance or leave to remain you had.
Shall I give my university Visa BRP reference in this box which is dated back in 2012? Because 2012 is the only time I sat for IELTS after that I never had to prove any English test.

Please help me understanding this. Many Thanks.
