URGENT - Self-Assessment Form Not Sent!

Do we have to take a PDF copy of the Self - assessment points-based calculator??? I mean, when we calculate our points using that online calculator, do we need to take a PDF copy of the points and submit it along with our forms??? Please, please help me with this!
