VAF 1 form - please help

As per VAF 2 form, now I have a working fillable using Adobe reader VAF 1 form.

Please let me have your views etc as to any bugs or problems

In both files the tabbing order may not be perfect, pls use the mouse for now

Once all the bugs if any are ironed out I'll work on the tabbing orders

SIGH if only the FCO would do this themselves and save us such a lot of hassle
