Query regarding form filling

Guyz i need the clarification of the following questions on HSMP form
1) On Page 2 In Box A we need to only enter email address or both email and postal address
2) Questions 18,19,20 only need to be filled if there is representative ?
3) What we write in Question 28, the country code?
4) What we write in Question 29, the income band? (well i put the band B for me)
5) There are boxes or fields which can't be fill in by typing but we need to fill by pen , so is it ok to go like this?
5) Whatever not applicable to me , i am just leaving them blank instead or writing N/A

You can keep it blank if applying without any representative; however not mentioned anywhere about this.

3) What we write in Question 28, the country code?
Dont know, am confused myself!!!

No problem, you can fill it by pen.

That sholud not matter

take the print of the form and fill it out by pen.

next Q28 refers to country code, which A,B,C,D,E
next Q31 refers to income---u have to write the figure eg: 15,313+

leave it blank, dont write N/A.
