Feedback? - Review request: US app refused for Past Earnings

Hi folks,

Any feedback on the draft below would be appreciated. I spoke with the caseworker who refused my app and he suggested that I include the page that I am calling "indicative documents page" with my review request. He also suggested that I emphasize in my letter the fact that the documents were previously submitted with my HSMP application.
