Q18, Q19, Q20 - Personal Contact Details or Contact Details of legal representative in the UK?

To be left blank if no legal representative in the UK?

Q28: Country Code

No country code is mentioned. Only list of countries classified into different Bands.

Q31: Income Band

Band A, B, C, D, E or £_____+

Can the form available on HO 's website be completed online, then printed and signed or the same has to be filled in applicant's handwriting?
