Query in completing Tier 1 Application form

I have couple of queries on completing the Tier 1 (General) application form. It would be great if someone can spare few minutes to clarify my doubts. Your reply would be of great help to me in filling the Tier 1(General) application form.

I'm in London on work permit and applied for HSMP in Feb-08 and got the HSMP approval letter on 07-May-08. My queries are

1. If I’m having a valid HSMP approval letter should I have complete the summary sheet in "Section 5" of the Tier 1 application (by completing the points claimed & documents provided)?

2. I’m sure I need to attach all original documents that I have attached with the HSMP application form. But while applying for HSMP I submitted pay slips for period Jan-07 to Dec-07. Now while applying for Tier 1 should I have to attach latest 12 months pay slips or is it enough if I attach same set of pay slips (Jan-07 to Dec-07) that I submitted for HSMP because Jan-07 is 16th Month if I apply now.
