Tier 1 (General) Dependant - child born in UK

I am in UK on Tier1(General) visa and I have got a baby girl before 2 weeks. I have few questions regarding filling up the application form and maintenance requirements.
1. Section C: Have u got entry clearance or leave to remain? as my child born in UK...she has not got any of this.
2. Section D: Immigration history - none of the questions applies to my child.. so what sould i put in?
3. Signature: should i put my name and signature on Declaration page?
4. Maintenace: how much fund shoudl I show for this requirement. As I am applying for her visa seperately.. do i have to show 533.. or 800 + 533 for three months.
5. I have got a cash ISA account and the statement i get is a yearly one issued on 8 th April.. the bank refused to give me a later home office require and can only give balance certificate.. can i attach a statement dated 8th April and a current balance letter dated 22th August to show my funds available?

I have read para 34 which says that letter showing balance on a particular day is not sufficient. But I have got a yearly statement to support the letter and because this is CASH ISA account, a withdrawn money can not be replaced, which proves that the money has been in the account for more than a year. Actually the bank has refused to give me recent statement as there is no trasactions and they said they can not give any specific letter other than a balance certificate. I am planning to write a letter explaining this situation.. will that be valid?

Shoud I sign the form on behalf of my child?

It also says that the statement should not be more than 1 month old at the time of submission - a condition that you are unable to fulfil.

If you have fully subscribed to the Cash ISA limit in a tax year, any amount withdrawn cannot be reinvested until the following tax year. But there is no restriction on withdrawing the "entire amount" or transferring it to another ISA.

Take an example - assuming you had 10K pounds in April (sum total of subscriptions from previous tax years, which your statement shows). Let's say that you withdrew the 10K (in full) in July-08 and then deposited 1500 pounds (of the 3600 pounds subscription allowed in current tax year) on 25-Aug. Do you think the statement dated April and the letter from bank showing EOD balance of 1500 pound on 25-Aug will satisfy UKBA, irrespective of whatever explanation you might try to provide???

Anyways I'll stick to what I said before that I don't think it will work ('coz honestly) it isn't UKBA's problem that you hold your maintenance funds in an ISA account and are unable to provide updated statements.

You might want to check with UKBA directly.

One final question: I am bit confused about maintenance funds, do i have to show only £533 for last three months or i have to show £533 for my child and £800 for myself?
