Application queries for self and dependent - Please help

Hi All,
I am applying for Tier1 from within UK. I have started filling the Application form for me and my dependent. I have few queries. can you please help me on this.

1. Please enter the reference number on the entry clearance or visa which can be found in the applicant’s passport or travel document.

--Is this the VAF no or the numbers in red printed on the Workpermit Visa

2. Section G -
G1.Does the applicant consider him/herself to be covered by the HSMP Forum Ltd. Judicial Review Judgment of 8 April 2008?

G2. Does the applicant wish to have the application assessed in accordance with paragraph 39-42 of the HSMP Forum Ltd. Judicial Review: Policy Document?

--I have never aplied for HSMP, so should i say no to both these questions?

Dependents Application:
1.Has the main applicant received notification that their application for leave to remain under the Points Based System has been successful?

- I (main applciant) am applying along with the dependants applciation. I guess this should be No for this.

and finally is it mandatory to submit the marriage certificate since i dont have it in with me in uk. They have not specified anywhere that we have to submit this. My wife's passport already has my name in the spouse column.

And in the main application there are pages which are not relevant to a fresh applicaiton( they are for extensaion applications) - can i remove them when sending? or should i keep all the pages in the application

Got the queries to my most of my question. Thanks very much.
But on a wiki page i saw the VAF no as the visa ref no. can you please confirm.

Can you please answer for these questions:

2.Can i remove the extra pages(pages relevant ot extension) or should i send the entire applciation set
