Hi,

I have come across a few doubts on the application form. I have listed down a few doubts so that everyone can benefit it.

1. Do i use Tier 1 general version 11/08 form for HSMP extension (NOT COVERED BY JR)

2. if you dont have any dependants or if there is any section that we dont need to fill, then do we take a printout of that section even though we dont fill it?

3. if we have to take printout then do we score that section or leave it blank

4. can i use debit card to pay. my debit card is solo. there is no option for solo card on the form at all

5. on page 6 there is question that askes "Home office reference number if known " - is that the reference number on my initial approval letter

6. on page 10 also Q B10 and B11 askes for Home office rference number and current or previous worker reference number. ? What do we fill here

2. Yes. Submit the whole form, even if certain sections will be blank.

3. Yes. leave the section blank if it is not relevant to you.

4. No. Use your credit card.

5. Yes.

6. As per your last question. If you do not have a worker reference number, leave the section blank.

i think you can pay by debit card

in the application form at page 5 it says

Paying by credit / debit card
A12 The name as displayed on the credit/debit card A13 Card number - this is the long number across the centre of the card

A14-A16 Enter the details where available on the card

A17 The Card Verification Value (CVV) is a three-digit security code, found on the back of the card on the signature strip - it consists of the last 3 digits.

A18 For card payments circle the amount you are required to pay
A19-A20 Cardholder’s signature - the person named on the credit/debit card must sign and date

On pgae 6, A4 askes for HO reference number - if this is the reference number on inital HSMP approval letter then again

on page 10, as i asked before QB10 askes for Home office reference number - is this also the reference number on inital HSMP approval.

could someone pls clarify this please?
do we use the reference number on initial hsmp approval twice

Q B11 askes for current or previous worker reference number under work permit arrangements - i suppose i can leave this since i am on HSMP

on page 48 Question T4 askes for earning period.

I am showing from 1/1/08 - 31/1/08. But during this period i have had 3 jobs. so do i mention as 3 different dates or just one date which is 1/1/08 - 31/1/08

Earnings period should be 12 consecutive months of the last 15months...

NO, Only credit cards accepted ( if UK).

I used my Solo card and there was NO PROBLEM. Payment went through and I was granted Tier 1 a couple weeks ago.

I applied withing the UK.

CAN I KNOW WHAT OPTION DID YOU TICK ON THE APPLICATION FORM FOR SOLO CARD. AS THERE is nooption for solo card.

also , on the application form in previous earnings section, it askes me toenter the dates of earnings. i am showing from 1/1/08 - 31/12/08. but have had 3 jobs during this period. so do i mention as 3 diff dates 9within 12 conceutive months) or do i just put as 1/1/08 to 31/12/08.
