Tier1 -which forms to be submitted

Dear all I wish ti apply for Tier 1 visa (Point based system). Please tell me which forms need to be submitted along with the certificate and proofs? I feel following forms are required :

1)POINTS BASED SYSTEM FORM (VAF9 NOV 2008) PERSONAL DETAILS
2)POINTS BASED SYSTEM FORM (VAF9 AUG 2008) APPENDIX 1: TIER 1 (GENERAL) MIGRANT SELF- ASSESSMENT
3) Self assessment point calculator .

Kindly guide me if I am filling wrong forms.

I think you're using the right forms.
