Doubts over IND Public Enquires response!

Hmm I am starting to doubt the IND Public Enquires now ... as far as I have been told by a few immigration companies in London, my situation falls into the HSMP Judicial Review (pre 2006 applicant).

This is the response I got ....

I'd be so grateful if you could answer my question. Just a Yes or No is fine

This is my situation:

HSMP
Date of Issue: 14 APR 2005
Valid Until : 12 APR 2006

HSMP Extension
Date of Issue: 28 FEB 2006
Valid Until : 12 APR 2009


As I applied for HSMP in 2005, and then my extension in 2006, I feel that I fall into the current category:

"Migrants who currently hold HSMP leave and have either applied for an extension of stay or will need to do so in the future"

Hence I would need to fill in this form:

Am I correct? Or should I fill in the Tier 1(General) further leave form?
