HSMP extension

Hi,

I wish to apply for HSMP extension to Tier 1.
Could somebody please let me know the list of documents that I need to submit along with the application form

1)Cover Letter
2)Salary Slips+Bank statements
3)Degree Certificate
4)Passport
5)Photos???
