dependent leave extension application

Hi

Could somebody advise me if there is a separate covering letter required for the dependent leave extension application?? I am attaching my bank statements for maintenance funds, is that going to be enough? or any other proof is required.. like marriage certificate?
And as I am going to pay for my dependent application, is it going to be ok if I enter my card details in the dependent application form?

I know these might be basic questions but I just wanna clear things

It is helpful for the caseworker to have a sight of a cover letter. Provide Marriage certificate birth certificates of dependant children.

If dependants are applying at the same time as the main applicant then a separate section in the main applicant's cover letter should also do.
