Doubt in Tier-1 Form Section-D Immigration History

Dear Friends,
I am working in UK and have got a valid HSMP Visa. I am applying for Extension of leave for me and my wife under Tier-1 PBS - General Category. I have got some questions and would highly appreciate help from you all.

About me:
I have been in UK since Aug 2004 with VISA statuses as below:
Aug 2004 - Feb 2005 Business VISA
Oct 2004 - Oct 2006 Work Permit
Dec 2006 - Dec 2008 Work Permit
Jan 2008 - Jan 2010 HSMP

About my Wife:
First came to UK on VISIT VISA from Dubai when she was single
Then she got Work Holiday Visa in UK
Then she went back to India
We got married and she came back to UK as my Work Permit Dependant
Currently she has got HSMP Dependant status as I've got my HSMP

In the form for Main Applicant,
Q1. Page 20, Section - Immigration History, D3-What date was the entry clearance or VISA issued?
- What should I write? Date as on my Business VISA when I came to UK for the first time or Date on my latest HSMP VISA as I am applying for extension of HSMP?

Q2. D4-Please enter the reference number on the entry clearance or visa which can be found in the Applicant's passsport or travel document?
- What should I write? VISA Number as on my Business VISA when I came to UK for the first time or VISA Number on my latest HSMP VISA as I am applying for extension of HSMP?

In the form for Dependant
Q3. Page 15, Section - Immigration History, D3-What date was the entry clearance or VISA issued?
- What should she write? Date as on my VISIT VISA when she came to UK for the first time or Date on her latest HSMP Dependant VISA as we are applying for extension of HSMP?


Q4. D4-Please enter the reference number on the entry clearance or visa which can be found in the Applicant's passsport or travel document?
- What should she write? VISA Number as on her VISIT VISA when she came to UK for the first time or VISA Number on her latest HSMP Dependant VISA as we are applying for extension of HSMP?
