Application for extension - HSMP -> Tier 1

I'm sorry if there's is already a thread for this...my eyes are crossed from reading some of the threads and the UK Border Agency website...

So I entered in with HSMP, and I need to extend my stay under Tier 1 (General)...can someone direct me to the forms and documents? Or is it basically the same thing I had to do in the first place? E.g. do I still need to prove language, degree etc?
