HSMP to Tier 1 Extension

I@m sure these questions are already asked and answered but I could not find any threads among 100's of search results.

Please help with the following questions.

What is the correct application fee for HSMP to Tier 1 main applicant?

On question d8 I have to supply the airline booking number or reference for my flight which I took two years ago. This is something I dont have. What do I write here?

on Page 53 question s5 - on clumn 5 do I need to write the sum on the box with the £ sign?
for example employer 1 15,000.00
employer 2 16,000.00
so in the second row column 5 inside the box with the £ sign do I write 31,000.00 ?

just wanted to be sure before I fill it in

Also do we have to include all the pages of this form as after page 30 we have to start on page 49 (Question 3C) ?
