K5 - Employer Contact details - Help Needed

Dear Friends,

In the application form -
Source of earnings (include details of employer; name of business etc and start/end dates for this source of earning)

Please let me know what details need to be provided - ?

include details of employer - Company name or Payroll Contact Person..?
Name of the business - Companny Name or Feild of Business..? such as IT

I am applying in person, can I give phone numbers only..rather than address (bcos not enough space to fill the address)

Do we need to provide HR contact details anywhere else in the application form..?
