Tier 1 Dependant Visa Withdrawl

Hi,

I have applied for my daughters PBS dependant visa on 29th nov 2010 and have not yet received any update from Home Office.I have to travel to India on 28 jan 2011 without fail and will be withdrawing application on Monday.My question are as follows:

1)Should I withdraw the application through phone or shall I visit the Croydon officce?
2)If am withdrawing in person than what form should I be filling and from where would I get the same?
3)Is there any previous experience in regards to the time line as they say 15 days on phone?
