How to inform UKBA for change of name

I am on Tier 1 General and had changed wife name. I have "affidavit for change in name deed poll/sworn affidavit", yet to change her name in visa.

1.Do I need deed poll from uk or above affidavit is sufficient?
2.Informing UKBA using "MCC POINTS BASED SYSTEM: MIGRANT CHANGE OF CIRCUMSTANCES FORM", will be sufficient or do I have to get new visa for her/or any alternative

Any assistance is highly appreciated.
