Query on the Dependant application form (Section 5)

Planning to send off my Tier 1 forms in a few days.

Whose name and sign should be there on the Section 5 "Consent to be given to UKBA before requests for verification are sent to the bank"

a. in case if dependant is an adult (say wife/partner)
b. in case if dependant is a minor.

Also it would be good to know which of the 3 in this section should be filled
