Dependent Tier 1 Visa Appointment (Document submission)

Hi All,
I got my tier-1 valid from jan 2010 and recently got a job offer in UK and would be moving shortly. I am in ireland at the moment.
Recently I applied online for my wife and child for dependent visa and got an appointment next week where we all 3 have to get to dublin embassy in person and submit all the documentation and necessary fee for dependent visa.

Could you please let me know if any document is missing from this list

1) My original passport with Visa stamped / Wife & Kid passports
2) Offer letter and contract of employment received from UK company
3) Marriage certificate
4) Child birth certificate
5) Bank balance of 6000 Pounds maintained for last 3 months (issued from bank in india )
6) Passport sized photographs of wife and kid.
7) Necessary Fee for wife and kid.

Should I fill up any other form again? Are all these sufficient.
