There is only one version of the form on the UKBA site. It's the latest version and that is the one that you must use.

Many thanks

When I applied for initial visa application I'm not sure whether I Claimed points for age or not.If I've claimed points for age,can I claim during extension.I think when I applied for visa initially earnings and education itself crossed more than 75 points.

Please can you advise on this.Is ther any way to check with uk border to find the points that I claimed for?

Thanks.

Where does it say that you cannot do now if you did not do it then?

There is a section in the form says as below.I got confused as I did not claimed the points for age at first grant of leave whether to tick the box or not.

G1. Please tick the relevant box to confirm the points claimed by the applicant on the basis of his/her age at the date of application for the first grant of leave in the category.

you can claim age points now for your extension. The age for claiming points during extension = your age at the time of your initial application, if this is your first application for extension
