Number of photos - Indian passport renewal

Hi, As per requirement of Indian passport renewal in London, I need to submit 4 photos of 2" * 2". There is one photo required on application form. Does this mean I need to have total 5 photos? or one of four needs to be put on application form.

Please advise urgently.
