Minor child passport re-issue

Dear All,

I am planning to get my minor son's passport re-issued.

If you have done this before, could you please advise me on the below?

1. Is it required to show proof for "change in appearance" from infant -> child? As you can understand there is quite a lot of change in 5 years (1 -> 6 years age).
2. Is there any special form to fill up as my son will be signing now instead of the thumb impression in old passport?
3. Your experience if you went through this process earlier?
4. Either me or my spouse will be going to the HCI London office to submit the application. Do we need a letter of authorisation from one of them (who is not going)? And also any letter from child?
