Surrender Indian passportand OCI of Indian after British citizenship

For adults OCI, bit urgent please.....much appreciated your help as tomorrow i have appointment

Kindly update me what certificates to be uploaded in OCI form as we have only 4 documents to be upload max

in that 4 documents what documetns to be loaded, in the below 5 is there???
did i miss any thing? do i need to upload my parents indian passport?

1. Current passport - british passport and naturalisation MERGED pdf
2. Indian orogin proof - cancelled passport and surrender ceritificate MERGED pdf
3. spouse passport - spouse passport and oci card MERGED pdf
4. marriage certiicate - marriage certificate and address proof MERGED pdf
5 Employment work letter - employee payslip and employment letter stating she is working - MERGED pdf
