Help in Filling OCI Application online PART-B

I am filling application for my wife.
I have completed PART-A and in PART-B its asking
we are five in our family.
myself British passport and OCI
my three kids also British Passport and OCI
now my wife got British Passport & we applying for my wife OCI....

Have you/any member of your family applied for Overseas Citizen of India registration earlier?

what I need to fill (yes/No), should all our four family members (myself and 3 kids) OCI details should be written...or she is applying first time so simply (NO) in check list box.. please suggest.


Have you surrendered/got your previous Overseas Citizen of India card cancelled?

my wife applying for first time OCI so how to mention YES / No.
