Reconsiderations

Hi , does anyone know (1) if UKVI acknowledge receipt of reconsideration requests (FLR(M) , (2) if it is possible to check if they have received a reconsideration letter (3) if so how to contact them to check (4) what the average decision time is for a reconsideration if it is a pretty clear case of mistakes on their side ? Any thoughts on this - many thanks ! BM
