I'm considering switching from my Tier 2 visa to the BNO visa and I already live and work in the UK. Will I need to pay for the healthcare surcharge again?

Yes, you will need to pay the IHS again if you switch to another visa type, even if you do that part-way through the current visa.

It is a "surcharge", over and above the charge, on all the relevant applications and it needs to be paid with every application submission.
