renewal of leave to remain

Hello,
My leave to remain in the UK is in 2 months time.

My 10 year route leave to remain is with my unmarried partner whois a British citizen.

This was achieved by my lawyer who argued with the human rights route.

I will like to do the renewal myself.

Please what is the right form to fill and submit?
