Spouse ILR Submission -Please HELP !!!

Hi All

Few concerns regarding my wifes ILR submission

1.Can you clarify if the date of submission is 28 days prior to the date stamped on the first entry to UK and not the date when visa was granted.

2.She is a housewife, so does not have any payments going into her bank accounts from work etc.

3.To show finances, will it be ok if I provide my personal Bank account statements.

4.Do I need to provide my P60/payslips OR is that not needed.

5.We have lived at 2 addresses over the past 1 year 11 months, so we have the following documents:
•Bank Statements for last 6 months in Husband Name @ Address 1 (Parents House)
•Bank Statements for last 6 months in Husband Name @ Address 1 (Parents House) – Another Bank
•Current Bank Statement in Wife Name @ Address 1 (Parents House)
•Current Bank Statement in Wife Name @ Address 1 (Parents House) – Another Bank
•Tenancy Agreement in Joint names @ Address 2 (Rented House)
•Council Tax in Joint names @ Address 2 (Rented House)
•Water Bill in Joint names @ Address 2 (Rented House)
•Utility Bill – GAS/ Electric in Wife name @ Address 2 (Rented House)
•Utility Bill – BT in Husband name @ Address 2 (Rented House)

Are the documents listed above sufficient or please advice what else is required.

Also please confirm if SET(M) version 04/2009 is the current form to be filed along with, 2 passport photos (wife) and 1 photo (husband); Passports for Both (husband and wife); Life in the UK test Pass Certificate (Wife).
