I have got my PEO appointment on the 27th and on the online PEO website there is a checklist before the appointment. One of the them is:

Print and sign the official form and bring it to your appointment. To do this, click Continue below.

When I click continue it takes me back to the original page and I cant see any form???

I believe they mean the application form, I printed the appointment email as well. You just print the PEO appointment notification only.
