Critical Skill Permit application

Hi Board members and the Team

Am IT professional working in UK and have been offered a JOB in Ireland.

My employer has now asked me to apply for Critical Skill Permit

Could you please advise whats the process of the application?

Is there anyone in the forum who have applied for the critical skill visa from themselves?

Note:
have gone through the sites and I get the basic idea of the application form, what am looking is more finer details, like what are the list of documents etc

I have got the info that I need to submit my colored scan copy of Passport (each and every page)....but am not sure..if its needed for each and every page or only the pages which are stamped
